Service models for stroke rehabilitation and recovery supporting transition from hospital to home are a trending topic. In this presentation research into the transition to home experience after stroke and the paradigm shift from hospital based to home based occupational therapy will be reviewed. Examples from the presenters work will be illustrated. Recently published work has sparked quite a bit of discussion around developing and testing the support of older people to maintain lifestyle and well-being when they must stop their occupation (e.g. driving,). It is concluded that applying an occupational lens is a key mean to apply evidence based practice and to enhance understandings of occupation and advanced occupational therapy.
